Disappearing toolbar button

In v.4.0.34, the `Attach a file' toolbar button has disappeared from the Message Editor window. What's more, it can't be reinstated - it isn't included in the `All Actions' list in the toolbars Customise dialog. I wonder why this has happened, please? Files can still be attached using a command on the Utilities menu. Many thanks.

Can't confirm, my v4.0.34 still has that button in the editor toolbar. And as I disabled most toolbars, my toolbars are in the default layout.
It's located in the 'edit' toolbar:
View -> Toolbars -> Edit

As for adding it to a toolbar, it's not easy to find, as the function isn't called 'attach' in 'All actions', but 'MIME-coded'.

Thanks, I've found the `MIME' button, renamed it and added it to the toolbar. By the way, attaching a file surely isn't an Edit action.
